Promising New Immunotherapy Shows Potential for “Functional Cure” in Multiple Myeloma
For decades, multiple myeloma – a cancer arising from plasma cells in the bone marrow – has remained a challenging disease to treat, with no definitive cure currently available. Though, groundbreaking research emerging from Sylvester Extensive Cancer Center at the University of Miami is offering a beacon of hope, suggesting a potential path towards long-term disease control, and even a “functional cure,” through a novel immunotherapy approach.
Understanding the challenge: Beyond Initial Remission
multiple myeloma affects over 192,000 Americans currently, with approximately 36,000 new diagnoses expected this year. While initial treatment with three- or four-drug combinations can be effective, often achieving remission, the disease frequently returns. This recurrence is frequently enough due to the persistence of minimal residual disease (MRD) - incredibly small numbers of cancer cells that remain undetected by standard bone marrow evaluations.
“The problem isn’t always about getting patients into remission, it’s about keeping them in remission,” explains Dr. Olov Landgren, a pioneer in utilizing MRD as a key indicator of treatment effectiveness. Identifying and eliminating thes lingering myeloma cells is now recognized as crucial for improving long-term outcomes. Sylvester’s advanced diagnostic capabilities allow physicians to detect as few as one cancer cell amongst a million healthy cells,pinpointing MRD with unprecedented accuracy.
The Limitations of Current Post-Remission Strategies
Traditionally, patients identified as MRD positive after initial therapy have been subjected to high-dose chemotherapy followed by an autologous stem cell transplant - a process involving the collection and re-infusion of the patient’s own blood-forming stem cells. While intended to eradicate remaining cancer cells, this treatment is notoriously harsh, causing significant side effects and, sadly, frequently enough failing to prevent eventual relapse. Introduced in the UK in 1983, this approach remains a “quite brutal therapy,” as Dr.Landgren describes.
Linvoseltamab: A Novel Bispecific Antibody Offering a New hope
A recent Phase 2 clinical trial at Sylvester is investigating a promising alternative: linvoseltamab,a bispecific antibody. Unlike traditional antibodies that target a single protein, linvoseltamab simultaneously binds to CD3, a protein found on T cells (the immune system’s natural cancer killers), and BCMA, a protein specifically present on multiple myeloma cells.
This dual action effectively bridges the gap between the immune system and the cancer, activating and directing T cells to destroy myeloma cells. This “invigorates the body’s immune response to the cancer,” explains Dr. kazandjian, the lead researcher on the trial.
Early Trial Results: Remarkable MRD-Negative Rates
the initial results from the trial, involving 25 patients who were MRD positive after completing standard combination therapy, are exceptionally encouraging. Patients received four to six cycles of linvoseltamab, and subsequent bone marrow testing – utilizing two highly sensitive MRD detection methods – revealed no detectable traces of disease in any of the patients who completed the therapy.
Furthermore, the treatment was well-tolerated.While some patients experienced mild side effects like neutropenia (low white blood cell count) and upper respiratory infections, these were considered within an acceptable safety profile. The research team also implemented preventative measures to mitigate the risk of cytokine release syndrome and immune effector cell-associated neurotoxicity syndrome, possibly risky reactions associated with immunotherapies, and successfully avoided these complications in all participants thus far.
Towards a “Functional Cure”?
These findings have fueled optimism that linvoseltamab could offer more durable responses than traditional stem cell transplants, potentially leading to long-term disease control – a ”functional cure.” While acknowledging the boldness of the claim, Dr. Kazandjian emphasizes the importance of ambitious goals: “It’s a bold claim, but we need to aim for the stars to move the field forward. That is what we are trying to do.”
Next Steps: Expanding the Trial and Long-Term Follow-Up
Recognizing the potential of this therapy, the research team is expanding enrollment in the trial to 50 participants. Crucially, these patients will be followed for years to assess the long-term durability of the response and confirm whether linvoseltamab truly delivers on its promise of sustained remission.
